Metered, reserved and settled per request.
Every managed (non-BYOK) inference request reserves an estimated cost against the account's Paymod Code balance before it is sent, then settles to the real usage once the provider responds. A request is refused with an insufficient-balance error rather than allowed to run over.
This wallet-style reserve-then-settle accounting is intentionally the same shape as the financial platform's own budget reservations, just scoped to model usage instead of transfers.
